- THE PROGRAM -
Imagine Art provides comprehensive, wraparound services in partnership with Health and Human Services to manage care for two long-term care waivers in Texas: The Home and Community Based Waiver and The Texas Home Living Waiver. Our waiver program focuses to empower families to navigate and manage their long-term care waivers.

PURPOSE
Long-term care waivers transform lives by empowering families to thrive in place! When delivered effectively, the waiver services offer a comprehensive array of services that support community based living. We creatively leverage this public resource for independent living in ways that are person centered and which engages peer support. STORY
Imagine Art established its long-term care services in 2016, after recognizing a deep need for systems change in how providers have historically delivered waiver services. Imagine Art aims to offer a personalized approach to offer a high caliber of services with an extensive network of therapists and attendants. Our network values person-driven approaches and self-advocacy.
SERVICES
Imagine Art supports individuals with IDD in community based settings including their own home, host (foster) home, or group home setting. Additional services provide community based support.
